Parking Ticket Appeal Form (PDF)
To use the Parking Ticket Appeal Form, download the form using Adobe Acrobat viewer and print out the form. The form should be completed and returned to the Kutztown Borough Police Department within twenty (20) days from the date the parking ticket was issued.
Business Emergency Notification Form (PDF)
The Business Emergency Notification Form is used by Kutztown Borough businesses and provides information to the police department so that we can contact someone in case of an emergency at that business. This form should not be used if the business has an Alarm Device. Alarm Devices require a permit. Go to the Alarm Device Permit page for more information.
Residential Vacation Notice Form (PDF)
The Residential Vacation Notice form may be used Kutztown Borough residents to report a vacant property due to vacation, death or other reason. The information provided on this form give the police department necessary information to permit us to contact the property owner in the event of an emergency.
Application for Transient Retail Business License (PDF)
Pursuant to Chapter 206 of the Code of the Borough of Kutztown, any person or persons engaging in the advertising, promoting, selling and/or explaining any product, service, or business organization, or soliciting orders for the sale of goods, wares, merchandise, or services, in persons, on public or private property, are required to secure a TRANSIENT RETAIL BUSINESS LICENSE prior to engaging in any of the above regulated activities. The fee for making application for a TRANSIENT RETAIL BUSINESS LICENSE is $50.00 and should be submitted to the Kutztown Borough Police Department with the completed application form and required additional documentation.
Request for Temporary No Parking Signs (PDF)
Residents may request the issuance of TEMPORARY NO PARKING SIGNS for posting at or near their residence for the purposes of moving to or from that residence, construction vehicles and equipment, wedding and funerals, and other related events. The REQUEST FOR NO PARKING SIGNS form must be submitted and approved by the Mayor of the Borough of Kutztown in advance of the date requested. Additionally, the TEMPORARY NO PARKING SIGNS must be posted by the requestor at least 24 hours in advance of the date and time requested and can not be posted for more than 72 hours.
** An individual or organization is entitled to (5) signs free of charge per calendar year. A fee of a $1.00 per sign will be charged for signs beyond this amount.
On-Street Person with Disability Parking Space Application (PDF)
